Temporary placards are only issued to those who have a temporary disability and are issued for 6 months. If necessary, placards may be renewed for an additional 6 months.
Placards may be used with any vehicle in which the handicapped person is a passenger, regardless of whether that person owns the vehicle.
Complete and print New Jersey Temporary Placard Application.
If you are unable to complete the application online, the form may be picked up at the Madison Police Department Records Bureau.
Ensure your doctor has signed the application in the doctor certification section of the form.
Return the completed application to the Madison Police Department Records Bureau with a check or money order made out to “NJ Motor Vehicle Commission” in the amount of $4.00.
