About Temporary Handicap Placards

  • Temporary placards are only issued to those who have a temporary disability and are issued for 6 months.  If necessary, placards may be renewed for an additional 6 months.
  • Placards may be used with any vehicle in which the handicapped person is a passenger, regardless of whether that person owns the vehicle.

How to Obtain a Temporary Handicap Placard

  • Complete and print New Jersey Temporary Placard Application.
  • If you are unable to complete the application online, the form may be picked up at the Madison Police Department Records Bureau.
  • Ensure your doctor has signed the application in the doctor certification section of the form. 
  • Return the completed application to the Madison Police Department Records Bureau with a check or money order made out to “NJ Motor Vehicle Commission” in the amount of $4.00.
