All marriage license applicants are encouraged to make an appointment by calling the Madison Health Department.

Ordering Vital Records & Certified Copies

In order to obtain a copy of a vital record, you must  submit the following:

• A completed application;
• A copy of the proof(s) of your identity;
• The correct fee (listed below);
• Proof of your relationship to the person listed on the vital record you are requesting (Only required for certified copies);
• Marriage Certificate, if name was changed due to marriage

Fees:

Fees are payable via cash or check (payable to the Borough of Madison) only. 

• $10 per Death Certificate;
• $20 for first copy of Birth or Marriage;
• $15 for each subsequent copy ordered at the same time
