REGISTRATION INFORMATION

Registration closes on Friday, 11/2 at 4:30 PM. Space for the program is limited and will be assigned on a first-come, first-served basis. Forms not returned by the 11/2 deadline may not be accepted and are subject to a late charge of 20%.

Please carefully review all items listed below before submitting your registration materials. Incomplete or incorrect forms will be returned. Please read all forms very carefully, making sure to sign and initial wherever required.

At a minimum, you must submit the forms contained in links 2 and 3. Please do not send anything directly to Mt. Peter. All items should be mailed or hand delivered to the Department of Recreation at 50 Kings Road.

1. Chaperone Forms (CLICK HERE)

  • If you would like to chaperone, please e-mail the Department of Recreation at recreation@rosenet.org to verify that there's an opening and complete the necessary forms.
  • All chaperones ski and ride free of charge. All fees are waived for the children of chaperones who are able to attend at least five trips. If you plan on meeting this requirement, simply write "CHILD OF CHAPERONE" on top of your child's registration forms and complete the chaperone forms. This 5-trip minimum can be split between 2 adult family members. Any fees paid to Mt. Peter are non-refundable.
  • Unless you've already done so for another program, chaperones must complete the online background check for coaches as well as the CDC's online concussion safety course. The online course takes roughly 20 minutes to complete and the background check costs $12.90. The check and course are good across all programs for one full year, unless the program has its own specific requirements.

2. Mt. Peter Registration Forms (CLICK HERE)

  • You will again pay Mt. Peter directly for this portion of the program. You can pay via check or credit card.
  • DO NOT SEND ANYTHING DIRECTLY TO MT. PETER.
  • All fees listed are for the entire program. You should not multiply any of the fees by the number of trips.
  • PLEASE MAKE SURE ALL RELEVANT FORMS ARE SIGNED AND INITIALED WHEREVER INDICATED.
  • AT A MINIMUM, YOU NEED TO SUBMIT THE FIRST 3 PAGES OF THE PACKET, INCLUDING THE LIABILITY RELEASE FORM ON PAGE 3.

  • Please make sure you complete and sign the Code of Conduct portion of the form. Any violations of the Codes of Conduct may result in removal from the program and jeopardize participation in other recreation programs as well. The chaperones and bus drivers are to be treated with the utmost respect at all times. It is very important that we clean up after ourselves at all times.

All forms should be mailed to the Department of Recreation at 50 Kings Road. You can also hand-deliver them to the Borough Clerk's Office in the Hartley Dodge Memorial Building Monday-Friday between the hours of 8:30 AM and 4:30 PM.
