The Madison Health Department permits and inspects all food establishments in the borough to safeguard public health and provide to consumers food that is safe, unadulterated, and honestly presented.
The food code establishes definitions, sets standards for food operations and equipment and facilities; and provides for food establishment plan review, permit issuance, inspection, and permit suspension.
Those food establishments that are opening for the first time, changing ownership, remodeling, converting, or adding a new operation must:

3. Contact the Building/Construction Department at 973-593-3064
4. Visit the Health Department to apply for a plan review (if required) and a Retail Food License Application
